Plan Details
Each plan is valid for 12 months after enrollment, at which point it must be renewed. Unused benefits do not carry over.
- Plan benefits are only valid at Great Smiles Dental Care
- The in-house loyalty plan is not insurance, but a discount through Great Smiles Dental Care
- Plan discounts cannot be combined with any other special offers or discounts
- Annual fees are nonrefundable and subject to change without notice
- Members using third-party financing will incur a 10% increase in fees
- Members will be charged $50 for missed appointments
- Members may be dropped without a refund for frequent cancellations, reschedules, or missed appointments
