That’s why we offer an in-house loyalty plan in Surprise that allows you to save money on the dental care you need to maintain a healthy, beautiful smile! We have a plan for everyone. Enrollment is conveniently available right here on our website!
Healthy Adult
- 2 routine cleanings
- 1-2 dental exams
- Digital X-rays (if needed)
- Oral cancer screening
- Fluoride treatment
- 1 emergency visit (if needed)
- 1 cosmetic consultation (if desired)
- Save $320!
Periodontal Patient
- 2-4 periodontal maintenance visits (schedule recommended by dentist or hygienist)
- 1-2 dental exams
- Digital X-rays (if needed)
- Oral cancer screening
- Fluoride treatment
- 1 emergency visit (if needed)
- 1 cosmetic consultation (if desired)
- Save $650!
Child
- 2 routine cleanings
- 1-2 dental exams
- Digital X-rays (if needed)
- Fluoride treatment
- 1 emergency visit (if needed)
- Save $320!
(For children 12 and under)
Each of our plans is valid for 12 months after enrollment. At that time, the plan must be renewed. Unused benefits do not carry over.
- Plan benefits are only valid at Great Smiles Dental Care
- In-house loyalty plan is not insurance but a discount through Great Smiles Dental Care
- Plan discounts cannot be combined with any other special offers or discounts
- Annual fees are nonrefundable
- Annual fees and plan costs are subject to change without notice
- Members using third-party financing will incur a 10% increase in fees
- Members will be charged $50 for missed appointments
- Members will be dropped from their plan without a refund for frequent cancellations, reschedules, or missed appointments
